ENROLMENT POLICIES & PROCEDURES FOR INCOMING FIRST YEAR COLLEGE STUDENTS

  1. GENERAL POLICIES

    1. No student shall be accepted for enrolment unless he/she presents the proper school credentials upon enrolment.
    2. A student is deemed officially enrolled after he/she has submitted his/her appropriate admission credentials; made an initial payment of his/her school fees and was allowed to attend classes bythe University.
    3. Attending classes without being officially enrolled shall NOT entitle the student any credit for any subject.
  2. PROCEDURES

    1. Proceed to N13 of St. Lorenzo Ruiz Bldg. for admission and accomplish the following:
      • 1.1. Submit below documents:
        • 1.1.1. Form 138 (Grade 12 Report Card)
        • 1.1.2. USL Placement Exam Result
        • 1.1.3. Good Moral Character Certificate
        • 1.1.4. Certificate of Live Birth from PSA
        • 1.1.5. Church Marriage Certificate if married
        • 1.1.6. Affidavit of Non- Cohabitation if single parent
        • 1.1.7. Two (2) pieces 2×2 I.D. picture with white background
      • 1.2. Fill out the Admission Form properly.
      • 1.3. Present and submit the completed admission form to the Admission Officer for approval.
    2. Proceed to the Office of the Academic Dean in-charge of the program/ course for encoding of the required subjects. Be sure to get a copy of the subjects enrolled.
    3. Proceed to the Accounting Office at Fr. Theophile Verbist Bldg. for payment of fees.
      • N11- St. Lorenzo Ruiz Bldg. – School of Education, Arts, Sciences and Health (SEASH)
      • N12- St. Lorenzo Ruiz Bldg. – School of Accountancy, Business and Hospitality (SABH)
      • Ground Floor- San Jacinto Bldg. – School of Engineering, Architecture, Interior Design and Information Technology Education (SEAIDITE)
    4. Return to N13 of St. Lorenzo Ruiz Bldg. for the production of Identification Card.
